Chronic Disease Management Program

Chronic inflammation plays a role in numerous conditions including rheumatoid arthritis, cognitive decline, certain cancers, heart disease and diabetes.  Inflammation is the body’s natural immune response to protect and heal itself, so acute inflammation can be a good thing.  For example, if you have tissue damage due to a cut or a burn, the body has an initial inflammatory response that fights this injury and returns the body back to “normal”.  However, prolonged chronic inflammation, where inflammatory cells do not shut off, prevents the body’s tissues from repairing, and also destroys healthy cells.  Research has shown an association of inflammation in the body with diet, exercise and other lifestyle choices.  Healthy lifestyle choices can dramatically decrease inflammation in the body.

Our Chronic Disease Management Program is based on optimizing the six pillars of Lifestyle Medicine which are Nutrition, Physical Activity, Stress Management, Restorative Sleep, Social connection and Avoidance of Risky Substances and helps individuals manage chronic conditions including joint conditions such as osteoarthritis, hypothyroidism, cognitive decline and cancer survivorship.  In our program we review your medical history followed by an evidence-based lifestyle habits assessment (90 minutes).  We give you general lifestyle recommendations and also generate a personalized lifestyle plan focusing on nutrition, physical activity and restorative sleep.  Follow up is done by one-on-one video visits, biweekly for 3 months, to support you and help you implement the lifestyle plan.  By optimizing healthy lifestyle behaviors, this program offers support to manage symptoms and improve overall health.  We offer an ongoing maintenance program, if desired, after you complete the Chronic Disease Management Program.

Our 3 month program includes:

  • Review of Medical History
  • Initial Comprehensive Lifestyle Medicine Assessment with complimentary basic labs – HgA1C, lipid panel, CRP (90 minutes)
  • General recommendations and protocol for wellness
  • Generation of Individualized Lifestyle Plan focusing on nutrition, physical activity, sleep, stress management
  • Food and lifestyle journaling/tracking
  • Physical Activity videos through the My Viva digital platform
  • Access to nutrition education, culinary demo videos, recipes, newsletters, wellness tips
  • Six 1:1 follow up video visits (every 2 weeks for 3 months) where we will discuss goals, successes, barriers, accountability.  We will update the lifestyle plan as necessary. (60 minutes)

Total Duration:  7 consultations over 14 weeks with an option to continue with our Ongoing Maintenance Program.
