Our Foundational Wellness Program is based on the six pillars of Lifestyle Medicine which are Nutrition, Physical Activity, Stress Management, Restorative Sleep, Social Connection and Avoidance of Risky Substances. In our program we do a thorough review of your medical history followed by an evidence-based lifestyle habits assessment. We give you general lifestyle recommendations as well as generate a personalized lifestyle plan focusing on nutrition, physical activity, sleep and stress management. Follow up is conducted through one-on-one video visits, biweekly for 3 months, to help and support you in implementing the lifestyle plan. We also offer an ongoing maintenance program, if desired, for after you complete the Foundational Wellness Program.
Total Duration: 7 consultations over 14 weeks, with an option to continue with our Ongoing Maintenance Program.
